Position Purpose:

  • FULL TIME - BENEFITS - M-F, 8AM-4:30PM

  • DAYS

  • Master Degree

  • Licensed Social Worker (NJ)

  • Responsible for providing comprehensive assessment and social services interventions in the Center and in the home and coordination of 24 hour care for LIFE St. Francis program participant.

  • Education - Master's Degree in Social Work from an accredited Graduate School of Social Work

  • Licensure/Certification - Current licensure as a Social Worker in New Jersey, either as an LSW or LCSW; Must have a valid driver's license, proof of insurance and have means of transportation.

  • Experience - Minimum of one year of post-master's social work in a health care setting. One year of experience required working with frail elderly population in acute, long term care, or community based setting
