Description

ESSENTIAL ACCOUNTABILITIES:

  1. Assess persons referred for admission to the ACT Team in order to determine their functioning status and service needs.

  2. Provide a written report based on the initial assessment including recommendations and, if services are indicated, a treatment plan.

  3. Develop treatment plans with other members of the ACT Team and the Consumers.

  4. Participate in home visits, emergency consultation services and provide recommendations and treatment plans and crisis interventions.

  5. As needed, provide consultation and training to persons, collateral's and/or team members.

  6. Provide a treatment note on all services rendered.

  7. Implement and monitor recommended treatment plan.

  8. Establish and maintain appropriate pharmaceutical formulary.

  9. Meet with Treatment Team on a regular basis to keep them current regarding treatment plan and be apprised of any issues that may affect it.

  10. Coordinates services with other clinical team members.

  11. Conduct community field based community visits. The ACT Program conducts 85% of their services in the community.

  12. Prescribe and monitor medication for all ACT Consumers.

  13. Monitor treatment provided by collateral(s)/ other team members by observing to them for the purpose of giving instructions, providing feedback and adjusting practices.

  14. Must be able to monitor persons' psychiatric needs through direct observation and when necessary make appropriate referrals for the psychiatric hospitalization.

  15. Required to participate and attend meetings/training's at various sites that require different routes to work site.

  16. Participate in OMH approved ACT orientations and trainings.

  17. Perform any other service provision tasks assigned by the Team Leader.

  18. Attend clinical staff meetings as requested.

  19. Maintain confidentiality at all times.

  20. Work schedules and locations subject to change.

Qualifications

REQUIREMENTS:

  • Registered and licensed to practice in New York State admitting privileges in targeted hospitals

-must have an M.D. from accredited institution

  • 2+ years experience diagnosing and treating a mental health population

  • Experience working with adults

  • Ability to work in multi-disciplinary team

  • Excellent documentation skills escripts
