Assumes responsibility for a collaborative process which facilitates implementation of the patient’s plan of care from pre-hospital to post-discharge by managing an assigned unit case load. This includes the ability to assess, plan, implement, monitor and evaluate options and services to effect and appropriate, individualized plan for the patient across the continuum of care. Using independent judgment and discretion, the continuum must be reassessed as an ongoing process for the individual being served by the case manager/social worker. The case manager/social worker utilizes clinical knowledge and competence, positive communication skills (both verbal and written), problem solving and conflict resolution techniques, ability to effect change, strong skills in assessment, organization, and time management. The case management process requires a focus on customer service skills, knowledge of setting appropriate and effective discharge plans that ensures optimal patient outcomes with consideration to payer requirements. Enforce the proper application of hospital policies and procedures regarding appropriateness and timeliness of clinical care given. Can effectively recommend that staff (either Medical or Nursing) be disciplined to the Department Chair when the care is below the standard level.

Qualifications

Minimum 3-5 years relevant clinical medical surgical, ICU or ER experience. 3-5 years experience in managed care performing utilization review and discharge planning or 3-5 years experience in acute care performing utilization review and discharge planning. PRI Screen preferred, CCM certification preferred. Licensure as Certified Social Worker (CSW) or LMSW highly preferred. Membership in a professional organization preferred. Qualities that include effective written and verbal communication skills, decision-making and problem solving skills, and comprehensive assessment skills.
