POSITION SUMMARY:ABI: The mission of the Acquired Brain Injury Residential Program is to provide a community-based residential program for individuals who have acquired brain injuries or traumatic brain injuries (ABI/TBI). The program provides access to comprehensive and coordinated physical, psychological, medical, vocational, and social services in a community-based living environment. It offers residents the opportunity to develop skills, and receive ongoing supports they require to function effectively in the community. Furthermore, it assists residents to establish supportive contacts and relationships in the community that will serve as andldquo;natural supportsandrdquo; for them.RESPONSIBILITYAssist in the daily operation of the Program.Utilize all required assessment tools to assess clientsandrsquo; functionality and mental health status, needs and preferences, and ongoing appropriateness of services.Assist clients implementing individual recovery plans. Coordinate recovery planning to include members of clientsandrsquo; treatment team and clientsandrsquo; natural supports. Conduct regular reviews of recovery plans and modify as needed.Demonstrate an ability to develop a basic level of competency for providing services to clients with co-occurring disorders.Ensure appropriate referrals and linkages are made to other parts of the mental health system and other community resources, including clinical, vocational, psychosocial, educational and natural supports. Provide advocacy as appropriate.Assist all Program clients in the areas of health and safety and in the acquisition of independent living skills, including personal hygiene, meal planning, cooking, cleaning, and money management.Provide rehab interventions as directed by resident recovery plan, including facilitating rehab groups.Supervise the administration of clientsandrsquo; self-medication protocols.Read and write in communication logs, encounter/progress notes, and other departmental communication tools to ensure understanding and adherence to changes in clientandrsquo;s recovery plans and/or client/programandrsquo;s needs or concerns.Provide crisis assessment and intervention, utilizing clinical and emergency back up as indicated. Communicate client needs with hospital emergency rooms and emergency personnel.Maintain and update, on paper or electronically, client case records with required encounter/progress notes documenting progress toward goals and objectives.Enter daily service activities into the applicable data base and insure the data accuracy.Complete duties outlined in positionandrsquo;s performance standards within compliance ratings.Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.May be required to use own vehicle on company business and to transport clientsPerform other duties as requiredRegular and consistent attendance is an essential function of this position.
